In order to help in school, parents are asked to complete a DBS Check (Disclosure and Barring Service)

Please email the School Office at 47office@streetsbrook.solihull.sch.uk and a link will sent to you.

Once you have completed the online part of the application (which takes approximately 10 minutes), you will be asked to provide some identification to support the application.  Three examples are:

• passport (in date)
• photo driving licence (in date)
• Council Tax Bill (current year) or utility bill within the last 3 months or bank statement within last 3 months

Council Tax Bill/utility bills//bank statements must include your name, please check this if they are set up jointly.
